Step-by-step instructions to apply for membership

There are several ways to find the Membership Application form.

  1. The first is directly from the CMF Home Page by clicking on the Membership link.
  2. The second is from the home page of the Mesothelioma Health Network. In the navigation menu on the left-hand side, click on “Participate in the Network”. Alternatively, you can click on “Join the Network” in the list of Quick Links in the navigation menu on the right-hand side of your screen. From there, click on the link to the Membership Application Form.

Complete the Membership Application Form making sure you fill in all the required fields as indicated by a red asterisk (*). Other fields are optional but may help customize your Health Network experience. Please indicate your interest in receiving email notifications or in having your contact information listed in the membership directory by clicking on the appropriate box. Scroll to the bottom of the page, confirm your agreement with the Terms of Service and that you are a real person, and then click Submit.

You will then receive a confirmation e-mail from the CMF with information on how to continue the registration process. If you do not receive an email, please check your Spam/Junk mail folders. If you still haven't received it, please contact us and we will assist you. Once you follow the instructions in your confirmation email, you will be able to Login with your username (or email address) and password you specified during registration.
